WAIVER OF LIABILITY HOLD HARMLESS AGREEMENT FOR MINOR
Description of Activity: RBHS Day of Service
Please read this form carefully and be aware in signing up and registering yourself and/or your minor child/ward for participation in the above described Activity and any activities associated therewith you will be waiving your rights to all claims for injuries you and/or your minor child/ward might sustain arising out of this Activity; and you will be indemnifying, holding harmless and defending Riverside Brookfield High School / District 208 and all Villages, Towns, and organizations, participating in the RBHS PTO Day of Service (and their officials, agents, employees, and volunteers), including the Villages of Brookfield, Riverside, North Riverside, the Frederick Law Olmsted Society, Salt Creek Watershed Network, and Cook County Forest Preserve District for any claims arising out of the participation of yourself and/or your minor child/ward under 18 years of age in the Activity.
In consideration of myself and/or my minor child/ward under 18 years of age being allowed to participate in the Activity, I recognize and acknowledge that there are certain risks of physical injury associated with the Activity. I agree to assume the full risk of injuries that I or my minor child/ward may sustain, as a result of participating in the Activity and all activities connected or associated therewith. I agree to indemnify, hold harmless and defend Riverside Brookfield High School / District 208 and all Villages, Towns, and organizations, participating in the RBHS PTO Day of Service (and their officials, agents, employees, and volunteers), including the Villages of Brookfield, Riverside, and North Riverside, the Frederick Law Olmsted Society, Salt Creek Watershed Network, and Cook County Forest Preserve District for any and all claims injuries, damage or loss that I and/or my minor child/ward may have against Riverside Brookfield High School / District 208 and all Villages, Towns, and organizations, participating in the RBHS PTO Day of Service (and their officials, agents, employees, and volunteers), including the Villages of Brookfield, Riverside, and North Riverside, the Frederick Law Olmsted Society, Salt Creek Watershed Network, and Cook County Forest Preserve District as a result of my participation and/or my minor child’s/ward’s participation in the Activity.
The invalidity or unenforceability of any of the provisions hereof shall not affect the validity or enforceability of the remainder of this Agreement.
I have read and fully understand the above Waiver and Release of all claims.

Permission to photograph and videotape participants is authorized by your signature on this waiver.
By signing this waiver, I understand that I and/or my child/ward may be photographed or videotaped at the RBHS PTO Day of Service. I understand that this may include posting of photos to Snapchat using a Snapchat filter created specifically for this event. I give permission for photographs and videotapes of myself and/or my minor child/ward to be used by Riverside Brookfield High School / District 208 and all Villages, Towns, and organizations, participating in the RBHS PTO Day of Service (and their officials, agents, employees, and volunteers), including the Villages of Brookfield, Riverside, and North Riverside, the Frederick Law Olmsted Society, Salt Creek Watershed Network, and Cook County Forest Preserve District in any and all publications, websites, press releases, brochures, and other promotional materials. Such photographs and videotapes will remain the property of the above mentioned entities. Please tell the photographer if you do not want to be photographed.
